What is HITCharts?

HITCharts consists of 101 sample, fictional, outpatient medical records.  These fictional medical records can be used for coding practice at the beginning & intermediate level, medical record review, medical terminology review, anatomy/physiology review, etc.  Categories of the medical records include Physician Office, Emergency Department, Observation, Ambulatory Surgery, Chemotherapy Infusion, Ophthalmology, Dermatology, Interventional Radiology and Podiatry.

Disclaimer – This fictional medical record content was compiled by HITNOTS, LLC, as well as several consultants.  Please be aware that the medical record content was developed for educational purposes, and is totally fictitious.  Any similarity to a real person, living or dead, is incidental.  The medical content of these records may not be totally clinically accurate, and therefore should not be used as an example of diagnosis or treatment.  The medical record content should never be substituted for actual medical treatment.

You will have an opportunity to code each medical record using both the ICD-10-CM and CPT® coding systems, as well as HCPCS, in some cases.  Feedback will be provided for each coded medical record, however, most of the medical records are now in the process of being coded. So, you are welcomed to enroll now to view the records, but know that if you enroll now, you will find that you cannot get coding feedback on all of the 101 records.

The Sample Fictional Medical Records will be available 24/7/365 for either 6 months or 12 months, depending on which plan you choose.  They can be accessed from your home computer or your mobile device, or anywhere that you can get the Internet.
